All referral types must meet the following 3 standard criteria:
- 18 years or older.
- Resident or registered with a GP practise in England.
- BMI ≥ 30kg/m² (adjusted to 27.5kg/m² in people of black African, African Caribbean and Asian origin).
Additionally, the following criteria must be met for each pathway:
ID starting with GP:
- GP referral.
- Must have a current diagnosis of either Diabetes +/- Hypertension.
ID starting with SR:
- Self referral.
- Must be an NHS employee.
ID starting with EC:
- Elective Care referral.
- Must be waiting for one of the following surgies:
- Knee and Hip replacement.
- Laparoscopic Cholecystectomy.
- Hysterectomy.
- Hernia Repair.
ID starting with GR:
- General referral.
- Must meet standard 3 criteria.
ID starting with MSK
- Musculoskeletal referral.
- Must have a diagnosis of Osteoarthritis of the knee or hip.
Exclusion criteria
Pregnancy- Individuals who are pregnant at the time of their IC cannot start the programme. If a member confirms that they are pregnant during the IC, please advise them that they are not able to start the programme whilst pregnant and can seek a new referral in the future. Select, “Completed” as the call outcome and then select “no” when asked if you wish to onboard the member.
If a member becomes pregnant during the programme, they can continue but should be advised that they should check whether this is OK with their Midwife/GP. The member will still need to have a weight goal activated and should input regular weights, but they should not focus on weight loss as a goal.
Individuals with an active eating disorder are NOT eligible to join and should be referred back to the GP.
